Your Responsibilities

As a Hospice Continuous Care LPN, you will provide skilled nursing support and compassionate bedside care for patients experiencing acute symptom management needs.

Daily Responsibilities Include:
  • Deliver direct patient care in accordance with hospice care plans

  • Monitor patient condition and promptly report changes to the RN case manager and hospice team

  • Administer medications and treatments as directed

  • Provide comfort-focused care and emotional support to patients and families

  • Assist with pain and symptom management interventions

  • Document patient care accurately and thoroughly

  • Collaborate with interdisciplinary team members to ensure coordinated patient care

  • Support patients and families with dignity, compassion, and professionalism throughout the overnight shift

Qualifications
  • Current Arizona LPN license in good standing

  • Current CPR certification

  • Hospice, home health, long-term care, or skilled nursing experience preferred

  • Compassionate and patient-focused approach to care

  • Strong communication and organizational skills

  • Valid Arizona driver's license and reliable transportation

  • Ability to work independently in a home setting

Education Requirements
  • Graduate of an accredited Practical Nursing program

  • High school diploma or GED required
