About the Role:

Join our dynamic team at Victorious Family Care Center as a Licensed Practical Nurse (LPN) in Bartlett, TN! In this role, you'll provide compassionate care to our patients while working in a supportive and collaborative environment.

Position Title: Licensed Practical Nurse (LPN)
Education: Active LPN license in Tennessee, Arkansas, or Mississippi
Experience: Minimum of three years of clinical experience, preferably within a hospital environment
Summary:
The Licensed Practical Nurse is responsible for maintaining a current and valid LPN license in the states of Tennessee, Arkansas, or Mississippi, with at least three years of relevant clinical nursing experience. Reporting directly to the RN Area Manager, the LPN plays an integral role in supporting client care by actively participating in assessment, planning, implementation, and evaluation activities.
Essential Job Functions:
  1. Complete scheduled monthly and quarterly client assessments, perform PRN assessments as indicated by changes in health status, and document all findings in accordance with VFCC policies and procedures.
  2. Assist with management of client medical records to ensure accuracy, completeness, and relevance, while upholding standards for security, accessibility, and organization in line with VFCC policy.
  3. Promote early identification and prevention of health concerns through routine client rounds, ongoing health assessments, and precise documentation and reporting of vital signs and clinical observations.
  4. Fulfill daily shift responsibilities according to VFCC standards, including maintenance of transfer documentation, provision of nail care, and meticulous record-keeping for menses, seizures, intake and output, and other required health tracking forms.
  5. Support the maintenance of adequate pharmaceutical inventories to ensure seamless continuity of care, as well as accuracy in medication ordering and receipt from the pharmacy.
  6. Administer medications and treatments as prescribed, strictly adhering to the "7 rights" of medication administration: right medication, dose, route, individual, time, texture, and position.
  7. Foster collaborative relationships with interdisciplinary team members and peers, contributing to a positive work environment.
  8. Review Medication Administration Records (MARs) for accuracy and completeness within group homes and office settings.
  9. Sign for, verify, and assign all medication received for Victorious Family Care Center.
  10. Distribute verified monthly medications to residential managers upon receipt from West TN Pharmacy.
Non-Essential Job Functions:

  • Participate in and/or conduct health-related training for facility personnel as directed by the Nurse Manager.
  • Facilitate effective communication, including both verbal and written documentationโ€”pertaining to client health monitoring and preventive care.
  • Provide support to Manager on matters directly related to client well-being.
  • Review all consults, recommendations, and MD orders, ensuring proper transcription of orders.
  • Attend at least 80% of all scheduled meetings and 100% of mandatory meetings, demonstrating professional engagement.
Additional Requirements:

Candidates must be at least 21 years of age, possess a valid driver's license for the state of TN, AR, or MS, and successfully complete all pre-employment screenings, including criminal background checks and drug testing.
