Job Summary:
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our skilled nursing team. The LPN plays a critical role in delivering high-quality care to residents by supporting daily nursing operations, coordinating with interdisciplinary teams, and ensuring each resident's individualized care plan is followed with excellence and empathy.
Key Responsibilities:
  • Provide direct nursing care to residents in accordance with their care plans
  • Administer medications and treatments as prescribed by physicians
  • Monitor residents' conditions and report any changes to the RN or physician
  • Document all nursing interventions and resident responses accurately and promptly
  • Assist with assessments, admissions, discharges, and transfers of residents
  • Collaborate with CNAs and other staff to ensure the highest quality of care
