Job description

We are in search of a professional, compassionate, and dynamic part-time Licensed Practical Nurse (LPN) to join our Fellowship Square team. The candidate should be devoted to enhancing the lives of our Assisted Living residents, dedicated to addressing their daily needs, and committed to maintaining their dignity and quality of life.

The purpose of the Licensed Practical Nurse (LPN) position is to provide wellness services to the residents in the Oasis Assisted Living facility in accordance with the current applicable Federal, State, and local standards, guidelines, and regulations and our established policies and procedures.


  • Records on a timely basis all medications/treatments administered, including reasons for and effects of PRN medications.
  • Documents on the nurse's notes significant observations regarding the resident's needs for and/or response to medications/treatment.
  • Stores medications in a safe manner and accounts for all controlled substances through accurate recording.
  • Orders, stores, and administers tube feedings.
  • Assists the manager in the planning, conducting, and scheduling of in-service training classes for caregivers, including on-the-job training.

  • Must be a graduate of an approved school for Licensed Practical Nursing or Registered Nursing.
  • Must hold a current, valid LPN license in the State of Arizona.
  • Must pass a pre-employment drug test and background check
  • CPR/First Aid card, fingerprint clearance card, and current TB test required.
  • Understand DHS rules.
  • Previous experience in a geriatric setting preferred.
  • Must be flexible and have a desire to communicate and work with seniors.   
  • Must be motivated, self-starter, with a passion for serving seniors.
  • Experience in an assisted living or long-term care environment preferred.
  • Schedule Availability: Part-time, flexible position on weekends with options to work additional hours.
