LPN/LVN (Licensed Practical Nurse)

Job Title: LPN/LVN (Licensed Practical Nurse)

City/State: Redding, California

Shift: 8 Hour Days, Monday to Friday, 08:00 to 16:00, no weekends, no holidays

Contract Length: 13 weeks

Start Date: 08/17/2026

Job Description:

Licensed Practical Nurse position focused on providing direct patient care in a home health setting. The role involves routine visits to patients, management of wound care, and coordination with the care team using HomeCare HomeBase as the electronic health record system.

Responsibilities:

  • Provide wound care, including management of wound vacs and ostomies.
  • Caring for patients with cardiac conditions such as CHF and pneumonia, and those undergoing IV antibiotics therapy with PICC line care.
  • Perform routine visits, handling up to 7 patient visits a day using a point system.

Requirements:

  • Basic Life Support (BLS) certification from the American Heart Association (AHA).
  • Valid driver's license and insurance.
  • Experience with HomeCare HomeBase (HCHB) electronic health records.
  • Experience in wound care, wound vacs, and ostomies.
