Licensed Practical Nurse (LPN) - Full-Time & Part-Time

Location: Farmer City, IL

Haven of Farmer City is seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to join our skilled nursing and long-term care team. As a valued member of our team, you'll enjoy a supportive environment, consistent scheduling, and opportunities for advancement.

Responsibilities:

  • Provide direct nursing care in a skilled nursing and long-term care setting

  • Administer medications, treatments, and IV therapy as prescribed

  • Assess, monitor, and document resident conditions and care plans

  • Collaborate with physicians, nurse practitioners, and interdisciplinary care teams

  • Supervise and support Certified Nursing Assistants (CNAs)

  • Ensure compliance with Illinois nursing standards and facility policies

  • Respond to changes in resident conditions and provide clinical leadership during your shift

Qualifications:

  • Active Illinois LPN license required

  • Graduate of an accredited nursing program

  • Long-term care, skilled nursing, or rehabilitation experience preferred

  • Ability to work every other weekend

  • Strong clinical judgment, communication, and organizational skills
