Job Title: Licensed Practical Nurse (LPN)
Location: Wellspring Skilled Nursing Campus - Monroe
Reports To: Nurse Supervisor / Director of Nursing (DON)
Position Type: Full-time or Part-time | Hourly
Wellspring is a statewide nonprofit dedicated to serving seniors, families, and children throughout Michigan. We are currently seeking a Licensed Practical Nurse (LPN) to join our Skilled Nursing and Rehab team in Monroe. In this role, you will provide compassionate, high-quality nursing care while supporting seniors in your local community.
Responsibilities
  • Provide quality nursing care that promotes resident rights, dignity, individuality, and freedom of choice.
  • Conduct daily resident visits to observe, assess, and document physical and emotional status.
  • Communicate effectively and respectfully with residents, families, visitors, peers, supervisors, and interdisciplinary team members.
  • Keep nursing leadership informed of resident conditions, changes in status, and other relevant clinical information.
  • Supervise nursing activities on the assigned unit and support staff in accordance with established policies and procedures.
  • Interpret, implement, and adhere to organizational policies and procedures.
  • Respond promptly to resident, family, and staff inquiries within established timeframes.
  • Follow all safety and infection control protocols, including proper hand hygiene and disposal of soiled materials.

Qualifications
  • Current Licensed Practical Nurse (LPN) license issued by the State of Michigan Board of Nursing (required).
  • Successful completion of an accredited Licensed Practical Nursing program.
  • Minimum of two years of nursing experience preferred (home health, medical, surgical, or critical care).

Schedule and Compensation
  • Employment type: Full-time or Part-time
  • Schedule: Night Shift (4:45 pm - 5:15 am)
  • Weekends: Rotating weekends required.
  • Pay rate: $32.90 - 36.40 hour, based on licensure
  • Shift Differentials: $3.00 per hour for Night Shift
