Description
Longleaf Hospice and Palliative Care is locally-owned with offices located in Atlanta, Covington and Roswell, offering end-of-life care to those with life limiting illnesses in the communities we serve. We believe that this very special care is best provided by professionals who live and work in the community they serve. At Longleaf Hospice and Palliative Care, we integrate and address the diverse, seen and unseen needs of patients and their families.
The Float Nurse acts as a fill in for nurse coverage for all Longleaf offices, responsible for the overall management of all nursing care for assigned patients.
As a Float Nurse, you will:
โ€ข Use health assessment data to support Plan of Care
โ€ข Triage calls and assist patient/primary caregiver with problem resolution. Counsel the patient and family in meeting nursing and related needs
โ€ข Make visits for symptom management and support as needed
โ€ข Modify patient treatment plans as indicated by patients' responses and conditions
โ€ข Maintain current, accurate, detailed reports in accordance with policy
โ€ข May monitor, record and report symptoms and changes in patients' condition to the IDG
โ€ข Provide health care instructions to the patient as appropriate per assessment and plan
โ€ข Obtain and receive physician orders as required
โ€ข Pronounce patients
โ€ข Other duties as assigned
Requirements
โ€ข Current license to practice as a registered professional nurse in Georgia.
โ€ข Current and continued CPR certification.
โ€ข At least one or two years of recent home care experience; hospice experience preferred.
โ€ข Good verbal and written communication and problem-solving skills.
โ€ข Responsible valid driver's license. Reliable, insured, automobile for making home visits required.
