Primary Care Provider (NP/PA)

Urban Indian Center of Salt Lake (UICSL) | Salt Lake City, UT

The Urban Indian Center of Salt Lake is seeking a compassionate and dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to provide high-quality, culturally responsive primary care services to American Indian and Alaska Native communities. Join a mission-driven team committed to whole-person wellness and improving community health outcomes.

Position Highlights

  • Full-Time, Exempt position
  • Monday-Friday, 8:30 AM-5:00 PM
  • Competitive salary: $99,971-$155,088 annually
  • Locum Pay Rate: $90/HR
  • Reports directly to the Medical Director
  • Collaborative, patient-centered outpatient setting

Responsibilities

  • Provide comprehensive outpatient primary care for patients across the lifespan
  • Evaluate, diagnose, and treat acute and chronic medical conditions
  • Perform patient assessments, medical histories, and physical examinations
  • Develop and implement individualized treatment plans
  • Provide preventive care, annual wellness visits, screenings, and immunizations
  • Manage chronic diseases, including diabetes, hypertension, and obesity
  • Order, interpret, and review laboratory and diagnostic studies
  • Prescribe medications and treatments within scope of practice
  • Educate patients and families on disease prevention and self-management
  • Coordinate referrals and collaborate with specialty providers and community resources
  • Work closely with behavioral health, nursing, and care management teams
  • Maintain accurate and timely electronic health record (EHR) documentation
  • Participate in quality improvement initiatives and interdisciplinary meetings
  • Deliver culturally respectful care that honors Native traditions and values

Qualifications

  • Master's degree or higher from an accredited Nurse Practitioner or Physician Assistant program
  • Active and unrestricted Utah APRN or PA license
  • National certification as NP or PA
  • DEA registration or eligibility preferred
  • One year of primary care experience preferred; qualified new graduates encouraged to apply
  • Strong clinical assessment, diagnostic, and patient management skills
  • Experience with electronic health records (EHR) systems

Preferred Qualifications

  • Two or more years of primary care or community health experience
  • Experience working in Tribal, Urban Indian, or Indian Health Service (IHS) settings
  • Commitment to culturally responsive, patient-centered care

This is a Locum to Perm Opportunity Locum Rate: $90/HR plus Travel and Housing Competitive salary: $99,971-$155,088 annually for Perm Monday-Friday, 8:30 AM-5:00 PM
