                              Registered Nurse– Hospice Case Manager 

Pride Health is hiring a Registered Nurse – Hospice Case Manager for a contract opportunity supporting a leading healthcare organization in Corpus Christi, TX.

Responsibilities Include:
•    Managing a caseload of hospice patients in home and facility settings. 
•    Completing patient assessments and coordinating individualized care plans. 
•    Providing compassionate end-of-life care while educating patients and families. 

Qualifications:
•    Active Texas Registered Nurse (RN) license. 
•    Previous hospice nursing experience preferred. 
•    Previous travel nursing experience preferred. 

Important Details:
•    Employment Type: 13-week contract 
•    Start Date: ASAP 
•    Schedule/Shift: Monday–Friday | 8-hour day shifts (between 8:00 AM–6:00 PM) |        40 hours/week 
•    On-Call Requirement: Backup on-call one evening/night during the week and one       weekend every 4–6 weeks 
•    Coverage Area: Approximately 60-mile radius from the Corpus Christi office 
•    Mileage Reimbursement: Mileage is reimbursed between patient visits. Mileage from home to the first visit and from the last visit back home is not reimbursed. 
•    Pay Range: $53.00–$57.00 per hour
