PRIDE Health is seeking a local contract nurse RN Hospice for a local contract nursing job in Corpus Christi, Texas.

                              Registered Nurseโ€“ Hospice Case Manager 

Pride Health is hiring a Registered Nurse โ€“ Hospice Case Manager for a contract opportunity supporting a leading healthcare organization in Corpus Christi, TX.


Responsibilities Include:
โ€ข    Managing a caseload of hospice patients in home and facility settings. 
โ€ข    Completing patient assessments and coordinating individualized care plans. 
โ€ข    Providing compassionate end-of-life care while educating patients and families. 


Qualifications:
โ€ข    Active Texas Registered Nurse (RN) license. 
โ€ข    Previous hospice nursing experience preferred. 
โ€ข    Previous travel nursing experience preferred. 


Important Details:
โ€ข    Employment Type: 13-week contract 
โ€ข    Start Date: ASAP 
โ€ข    Schedule/Shift: Mondayโ€“Friday | 8-hour day shifts (between 8:00 AMโ€“6:00 PM) |        40 hours/week 
โ€ข    On-Call Requirement: Backup on-call one evening/night during the week and one       weekend every 4โ€“6 weeks 
โ€ข    Coverage Area: Approximately 60-mile radius from the Corpus Christi office 
โ€ข    Mileage Reimbursement: Mileage is reimbursed between patient visits. Mileage from home to the first visit and from the last visit back home is not reimbursed. 
โ€ข    Pay Range: $53.00โ€“$57.00 per hour
