Job Title: Licensed Clinical Social Worker (LCSW/LMSW) – Emergency Department

Location: Delaware (State License Required Upon Acceptance)

Position Type: Full-Time (Evenings or Nights) | Permanent

Overview:

We are seeking compassionate and experienced Licensed Social Workers to join our Emergency Department team. This role is ideal for energetic, motivated professionals who value care coordination and thrive in a collaborative environment focused on excellence and patient-centered care.

Key Responsibilities:

  • Collaborate within the Care Management Triad Team Model:
    • RN Case Manager – Drives patient care and discharge planning.
    • Social Worker – Addresses psycho-social barriers and discharge support.
    • Utilization Management – Ensures appropriate patient status and payer coverage.
  • Conduct psycho-social assessments and interventions.
  • Support patients and families during crisis situations.
  • Coordinate discharge planning and referrals to community resources.
  • Work closely with interdisciplinary teams to ensure continuity of care.

Qualifications:

  • Education: Master’s degree in Social Work (MSW) or Master of Social Services (Client) from an accredited institution.
  • Licensure: Must hold LCSW or LMSW license in the state of Delaware prior to start date.
  • Experience: Minimum of 2 years in acute care social work; Emergency Department experience preferred.
  • Specialty: Maternal-Child candidates must have experience with neonates, children, and families.
  • Certification: CCM preferred within 18 months of eligibility.
