Job description

Program: Woodycrest | 984 Woodycrest Avenue, Bronx, NY 10452
What You'll Do
As a Clinical Case Manager with HELP USA's Woodycrest Supportive Housing Program, you'll provide case management and crisis intervention services to residents with the goal of promoting independent living skills, increased housing stability, and well-being.
Your responsibilities will include:
  • Conducting assessments, formulating individualized service plans (ISP), and providing referrals and ongoing support to assist residents in developing skills and achieving personal goals associated with housing stability and well-being.
  • Serving as the foundation of the supportive services made available to residents such as connecting them to community-based services and resources that are aligned with their ISP.
  • Facilitating a range of support groups and workshops designed to help residents live as independently as possible.
  • Serving as a liaison with medical, mental health, and substance abuse providers to ensure the level of care that the residents deserve.
  • Providing assistance with educational and employment linkages.
  • Helping residents overcome barriers that may affect the timely submission of monthly rent.
  • Working with property management to support residents in all aspects of maintaining permanent housing.
  • Advocating for and with residents in all aspects of their lives and offering (but not imposing) assistance.

You're a great fit for this role if you have:
  • Master's Degree in Social Work or a related field.
  • At least two years of experience delivering clinical crisis services.
  • Experience in case management, assessment, counseling, and crisis intervention.
  • Computer literacy, specifically with Microsoft applications.

It would be helpful (but not required) if you have:
  • Experience in Motivational Interviewing (MI), Wellness Recovery Action Plan (WRAP), and Person-Centered Planning (PCP).
  • Knowledge of AWARDS and/or CARES.
  • CASAC certification.

Compensation
Actual compensation will not be less than the applicable minimum wage, prevailing wage, or exempt salary requirements under federal, state and local laws.
