Job description

Position Overview

The Case Management Coordinator (CMC) is responsible for collaborating and informing the case management process to facilitate appropriate healthcare outcomes for members. The role involves providing care coordination, support, and education to members through the use of care management tools and resources in the ABH-Louisiana Care Management Program.

Responsibilities

Evaluation of Members:

  • Use care management tools and data review to conduct comprehensive evaluation of referred members' needs and eligibility.
  • Recommend an approach for case resolution or to meet needs, considering benefit plans and internal/external programs.
  • Identify high‐risk factors and service needs that may impact outcomes and refer to clinical case management or crisis intervention as needed.
  • Coordinate and implement assigned care plan activities and monitor care plan progress.

Enhancement of Medical Appropriateness and Quality of Care:

  • Consult with case managers, supervisors, Medical Directors, and other programs to overcome barriers to meeting goals.
  • Present cases at case conferences for multidisciplinary review to achieve optimal outcomes.
  • Identify and escalated quality of care issues through established channels.
  • Utilize negotiation skills to secure appropriate options and services to meet member benefits and healthcare needs.
  • Use motivational interviewing to ensure member engagement and promote lifestyle/behavior changes.
  • Provide coaching, information, and support to empower members to make independent medical and healthy lifestyle choices.
  • Help members actively participate with their provider in healthcare decision‐making.

Monitoring, Evaluation and Documentation of Care:

  • Apply case management and quality management processes in compliance with regulatory and accreditation guidelines and company policies.
  • Maintain accurate documentation of care activities and outcomes.

Required Qualifications

  • Minimum of 2+ years' experience in behavioral health, social services, or a related field focusing on program management.
  • Minimum of 2+ years' experience with adult and/or adolescent populations.
  • Must reside in Louisiana and have reliable transportation; willingness to travel 50% of the time or more.
  • Mileage reimbursed per company policy.
  • Proficiency with personal computers, keyboard navigation, mouse usage, and MS Office Suite (Teams, Outlook, Word, Excel, SharePoint).

Preferred Qualifications

  • Experience with Medicaid and waiver programs.
  • Crisis intervention skills.
  • Managed care / utilization review experience.
  • Familiarity with QuickBase case management and discharge planning.

Education

Minimum of a Bachelor's degree or a non‐licensed master level clinician required, with preference for degrees in behavioral health or human services (e.g., psychology, social work, marriage and family therapy).

Weekly Hours

Full‐time, 40 hours per week. Monday‐Friday standard business hours 8:00AM – 5:00PM. No nights, weekends, or holidays.

Pay Range

The typical pay range for this role is $21.10 – $36.78 per hour, based on experience, education, geography, and other factors. Additional compensation may be available through bonus, commission, or incentive programs.
