PHS Rehabilitation is seeking a Part Time experienced Speech Language Pathologist, CCC-SLP certification required, to serve in our assisted living and independent living in Waukesha, Pewaukee, Oconomowoc, and Muskego area.
SCHEDULE: M-F, working up to 20 hours/week

Responsibilities
As a Speech Language Pathologist you will be responsible to:
  • Conduct a comprehensive assessment & evaluation using subjective and objective measures.
  • Develop treatment plans and programs in accordance with physician orders.
  • Provide quality treatments, actively progressing goals to achieve highest quality of life.
  • Conduct screens and provide education to residents/clients, families, caregivers, and staff.
  • Coordinate seamless discharges utilizing the full continuum of therapy and fitness services.
  • Provide accurate and timely documentation & billing in electronic medical record.
  • Demonstrate understanding and knowledge of payer sources and regulatory requirements.
  • Complete and document supervisory visits per regulations.
  • Communicate effectively with residents/clients, families, caregivers, physicians, referral sources, and interdisciplinary team.

Qualifications
  • Graduate from an accredited Speech Language Pathology program.
  • Current licensure in the state(s) of practice as required.
  • Demonstrated compatibility with PHS's mission and operating philosophies.
  • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
