Job Summary and Responsibilities

As our Patient Logistics RN, you will orchestrate all transfer center activities for CommonSpirit Health facilities. You'll collaborate with hospital administration, nursing leadership, medical staff, physicians, and transport vendors to ensure seamless patient transfers, acting as the crucial liaison between referring and Dignity Health facilities for admissions.

Every day you will actively engage in all transfer-related activities, meetings, and education, facilitating patient movement from entry to acute hospital settings. You'll precisely designate facilities based on patient diagnoses, planned procedures, and required level of care to optimize placement and ensure efficient patient logistics.

To be successful in this role, you will: possess strong coordination and communication skills, effectively managing complex patient transfers across multiple entities. Your ability to accurately assess patient acuity for optimal facility designation, coupled with a commitment to seamless transitions, is paramount for this critical coordination role.

Job Requirements

Required

  • RN with three (3) years of clinical experience in hospital setting with at least one year to include emergency and/or critical care setting
  • One (1) year recent acute care experience required.
  • Background in Utilization Management and a working knowledge of health care reimbursement
  • Experience using electronic bed management systems, preferably TeleTracking Experience in Patient Flow management
  • Working knowledge of computer programs such as Google Business Suite
  • Competent in select psychomotor skills
  • Knowledge of current nursing technique, principles and practice
  • Current AZ State RN license in good standing required.

Preferred

  • Background in Neurology/Neurosurgical and Cardiology
