Shift: 7:00am - 7:30pm

Job Description:

PATIENT POPULATION: The patient population served can be all patients including geriatric, adult, adolescent, pediatric, and newborn. This also includes services which affect facility staff, physicians, visitors, vendors and the general public.

POSITION SUMMARY: Reporting to the Director of Care Integration, the RN Transfer Center Coordinator is responsible and accountable for the act of coordinating both internal and external patient care needs by facilitating patient placement in the appropriate internal nursing unit or external hospital transfer utilizing pre-established criteria and consulting with members of the interprofessional team. Adheres to National Patient Safety Goals as appropriate based on level of patient contact this position requires. With hundreds of requests each year, the RN is the link to patient-centered care using teamwork, inter-disciplinary communication and collaboration to provide dynamic environment for ongoing development of the internal and external movement and placement of patients. This role is essential to the Command Center structure at SVMC. Must be able to work normal/scheduled working hours to include Holidays, call-backs, weeknights, weekends, and on-call. Agrees to participate, as directed, in emergencies and community disasters during scheduled and unscheduled hours. As a designated disaster service worker you are required to assist in times of need pursuant to the California Emergency Services Act. (Gov't. Code ยงยง 3100, 3102) Needs to recognize that they have an affirmative duty and responsibility for reporting perceived misconduct, including actual or potential violations of laws, regulations, policies, procedures, or this organization's standards/code of conduct. The employee shall work well under pressure, meet multiple and sometimes competing deadlines; and the incumbent shall at all times demonstrate a cooperative behavior with colleagues and supervisors.

EDUCATION/TRAINING/EXPERIENCE: Individuals should have experience working in the critical or emergency care setting and have an understanding of how to navigate complex medical systems with a focus on patient centered-care. Should have experience engaging with providers from across the central valley region, state of California and beyond. Requires an understanding of diverse patient populations which include medicine, general & specialty surgical services, neurology/neurosurgery, oncology, gynecology, urology, orthopedics and more for patients from birth through geriatrics.

Education: To perform this job successfully, an individual must have and maintain current licensure as a Registered Nurse in the state of California. Anyone hired on or after 7/1/2023 must already have their BSN or enroll in a BSN program within 30-days of start date and complete program within 24-months from enrollment. MSN degree is preferred. Prefer two (2) years of experience as a R.N. Case Manager. Must have and maintain current BLS certification or obtain within orientation process. ICU and/or Emergency Services (emergency department, fixed, rotor and/or ground transport RN) experience preferred.

Experience: - Two (2) years or more experience as a R.N. in an Acute Care setting required. - Experience with initiating and implementing patient transfers is strongly preferred. -Working knowledge of Excel and proven computer skills -Knowledge and understanding of EMTALA Regulations -Multi-task and proven organizational skills, demonstrating the ability to properly prioritize as needs shift. -Demonstrate initiative, resourcefulness and analytical skills in organizing and prioritizing patient data to expedite critical transfers in and out of Sierra View Medical Center -Be sensitive to nursing and department needs while also fulfilling patient and hospital needs -Communicate in a professional manner, demonstrating the ability to work as a team. -Excellent verbal and written communication skills -Work with little supervision

LICENSURE/CERTIFICATIONS: To perform this job successfully, an individual must have current licensure as a Registered Nurse in the state of California, and maintain BLS certification. Anyone hired on or after 7/1/2023 must already have their BSN or enroll in a BSN program within 30-days of start date and complete program within 24-months from enrollment. MSN degree is preferred. A national certification in a specialty area of nursing is preferred. Certified Emergency Nurse (CEN) or Critical Care Registered Nurse (CCRN) preferred.

Responsibilities and Essential Functions: *Indicates Essential Function 1 * Performs telephonic patient triage, initiating and monitoring admission or transfer communication with physicians, patients, and others in the healthcare continuum. 2 * Coordinates with outlying regional physicians and hospitals assisting with SVMC physician consultation, transfer, and coordination of patient ground or air transport. 3 Collaborates frequently with the ED, PACU, inpatient nursing units, care management, and the bed board to keep abreast of possible admissions/transfers and transfusions. 4 Collaborates with the bed board placement staff, nursing supervisor, Manager of Emergency Services and Chief Nurse each shift to determine census and acuity across the organization. 5 Collaborates and problem solves regarding bed availability, patient acuity/needs, and problematic situations with the RN Clinical Manager Manager of Emergency Services/Manager of Care Integrations/Nursing Supervisor/Chief Nurse. 6 Receives/obtains the admission orders for admissions and ensures that orders are sent to the appropriate nursing unit, including the management of the Direct Admit Line. 7 Advises the physician when information is obtained that may change the bed request, ETA, or treatment upon arrival. 8 Maintains the physician order(s) on file. 9 Assists with the orientation of new employees. 10 Works cohesively and collaboratively with the Command Center Operations staff. 11 Works closely with members of Care Integration facilitate inter-departmental communication and the coordination of post acute referrals 12 Authorizes all outgoing non-emergency transportations as needed and utilizes appropriate use transportation services
