RN Summary

  • The Registered Nurse (RN) provides skilled nursing care to clients in their homes in accordance with the plan of care, physician orders, and agency policies.

  • The RN performs comprehensive assessments, delivers evidence‑based interventions, documents accurately, and collaborates with the interdisciplinary team to ensure safe, high‑quality care.

LPN Summary

  • The Licensed Practical Nurse (LPN) provides direct nursing care to clients in their homes under the supervision of an RN and in accordance with the plan of care.

  • The LPN supports clients with skilled tasks, monitoring, and education to promote safety, independence, and improved health outcomes.


Certified Nursing Assistant (CNA) Summary

  • Perform personal care tasks aligned with SC CNA scope of practice.

  • Take and record vital signs, weight, and intake/output.

  • Assist with range‑of‑motion exercises and mobility programs.

  • Support wound care and skin integrity monitoring (non‑skilled tasks).


Home Health Aide (HHA) Summary
