Overview

Responsibilities
  • Perform focused patient assessments within scope of practice
  • Monitor and document vital signs, pain, intake/output, and clinical changes
  • Provide basic nursing care (bathing, toileting, repositioning, ambulation)
  • Assist with activities of daily living (ADL's)
  • Reinforce patient comfort and safety measures (fall precautions, skin integrity)
  • Administer oral, topical, subcutaneous, and intramuscular medications within scope of practice
  • May administer IV medications/fluids after completion of mandatory IV class offered during orientation
  • Perform wound care (dressing changes, sterile technique).
  • Carry out ordered treatments (catheter care, suctioning, tube feedings)
  • Observe for changes in patient condition
  • Report abnormal findings promptly to the RN and or provider
  • Document accurately in the EMR
  • Participate in early recognition of deterioration (sepsis, stroke symptoms)
  • Actively participate in Code Blue and Rapid Responses on unit as needed
  • Collaborate with RNs, CNAs, physicians, therapists, and case managers
  • Communicate effectively with patients, family, RNs, LPNs, providers, ancillary, CNAs and case management
  • Collaborate with RNs, CNAs, physicians, therapists, and case managers to meet patient care needs
  • Implement components of the nursing care plan developed by the RN
  • Delegating task to CNA's when appropriate
  • Participate in shift reports, bedside handoff, and huddles
  • Reinforce education provided by the RN or provider
  • Inform patient on medications being administered and related side effects
  • Maintain timely, accurate, and complete documentation
  • Ensure all care aligns with the State Nurse Practice Act, Facility policies, and Regulatory standards (Joint Commission)
  • Understand that documentation is a legal record of care and maintains HIPPA and patient confidentiality
  • Adhere to standard and transmission-based precautions per infection control policies
  • Perform hand hygiene and PPE use
  • Monitor for and help prevent hospital acquired conditions
  • Support patient safety goals aligned with quality measures
  • Assist with room preparation for new admissions and keeping patient rooms tidy throughout shift
  • Assist with specimen collections as ordered (urine, stool, sputum, and would cultures)
  • Contribute to workflow efficiency in high-acuity environments
  • Assist with postmortem care when required.
  • Assist with patient transport (discharge area, etc.)
  • Support organizations quality outcomes and patient experience measures
  • Completes all mandatory department, educational and hospital requirements
  • Adheres to current Infection Control and Safety Standards
  • Regular and prompt attendance

Additional Information

Employees must be in a regular position, working 20 hours or more per week (.50 FTE or greater) to qualify for benefits.

Qualifications
  • Graduation from an accredited school of practical nursing meeting the minimum standards set by the State of Alabama Required
  • LPN - Licensed Practical Nurse - State Licensure - Current Alabama Licensed Practical Nurse (LPN) license or current Multi-State LPN License in accordance with the Nurse Licensure Compact (NLC) for Alabama Upon Hire Required and
  • Basic Life Support (BLS) issued through on organization that requires a hands-on instructor lead psychomotor skills verification (ex. American Heart Association or Red Cross). within 30 Days Required and
