Position Summary

Coordinates all case management activities with members to evaluate medical needs and to facilitate the overall wellness of members. Develops strategy to address issues to outcomes and opportunities to enhance member's overall wellness through integration. Instructs programs and procedures in compliance with network management and clinical coverage policies.

**Must live in the following parishes in Louisiana**

• Region 1 (Orleans): New Orleans and surrounding parishes - Jefferson, Plaquemines, St. Bernard.

• Region 2 (Baton Rouge): East/West Baton Rouge and surrounding parishes - Ascension, Iberville,

• Region 3 (Covington/Houma-Thibodaux): Southeast coastal parishes - Lafourche, Terrebonne, St. Charles, St. James, St. John

• Region 4 (Thibodaux/Lafayette): Acadiana area - Lafayette, St. Landry, Acadia, St. Martin, Iberia, Vermilion, Evangeline, St. Mary

• Region 5 (Lafayette/Lake Charles): Southwest parishes - Calcasieu, Beauregard, Allen, Cameron, Jefferson Davis

• Region 9 (Monroe/Northshore): Florida Parishes - Livingston, St. Helena, St.Tammany, Tangipahoa, Washington

Minimum Requirements

  • Current/Active Clinical Licensure (RN, SW, LCSW, LPC, LMFT)

  • Case Management experience

  • Strong customer service orientation and problem solving

  • Excellent Communication/Telephonic skills

  • Excellent motivational interviewing skills/ability to built rapport and trust telephonically

  • Excellence in documenation and respecting compliance/regulatory standards

  • Highly organized, remain self-driven without direct supervision

  • Manage time effectively

  • Achieve performance metrics

  • Technology proficiency

  • Ability to travel within 125 miles round trip for 1 day in the field

  • Variable work scheduled with ability to work 2 days weekly until 9pm local time

Preferred Qualifications

  • Bachelors and/or Master's Degree

  • CCM Certification

  • Previous experience in Managed Care Organization

Pay Range

The typical pay range for this role is:

$54,095.00 - $116,760.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
