About Hopscotch Primary Care

At Hopscotch Primary Care, we believe great healthcare should be accessible to all people across all communities.  Today, almost 20% of Americans live in a rural community, yet only 11% of physicians practice in those same communities.  We are on a mission to transform healthcare in rural America.  We provide high-quality primary care tailored to meet the needs of our patients through our robust care model and comprehensive care team, delivering care in our clinics, and across settings, and wrapping resources around the patients who need them most.

Our patients and the care teams who serve them sit at the center of everything we do at Hopscotch.  Hopscotch Primary Care takes a team approach to serve patient needs and provide the best care possible.  Our goal is to provide the care each of us would want for ourselves or for our family members, in the right setting, and at the right time.

Today, we are serving thousands of patients in our value-based care model and the number is growing every day.  If you want to bring your experience, skill and passion to make a lasting impact in healthcare, we'd like to meet you.

ABOUT THE ROLE

We are looking for a Product Engineer, Care Delivery to help design and build the next generation of operational and AI-powered tools that support care delivery across our clinics.

This role sits at the intersection of care model operations, product management, and the data and AI platform teams. You will work closely with frontline clinical and operational teams to understand workflows, identify opportunities for improvement, and rapidly build software solutions that enhance how care is delivered.

You will leverage shared data infrastructure, platform capabilities, and AI infrastructure developed by our data and AI platform teams to create workflow applications and operational tooling that drive meaningful outcomes for frontline care teams.

The tools you build may include:

  • Care team-facing workflow applications that support value based care initiatives
  • AI-assisted tools that automate administrative work in clinics or enhance clinical decision-making
  • Embedded workflows within our EHR ecosystem
  • Lightweight standalone applications that improve operational efficiency
  • Internal tools that help care teams coordinate, prioritize, and deliver longitudinal patient care

This is a highly cross-functional, high-agency role for someone who enjoys moving quickly, working closely with end users, and building thoughtful software that solves real operational problems.

You will partner closely with Product Management on prioritization, workflow discovery, and problem definition, while maintaining significant ownership over technical design, implementation, and delivery.

RESPONSIBILITIES

  • Partner closely with care teams, operational leaders, and product managers to understand workflows and identify opportunities for improvement
  • Design, build, and deploy high-quality applications and workflow tooling that support clinic operations and care delivery
  • Leverage internal platform capabilities, data assets, APIs, and AI infrastructure to rapidly prototype and productionize new solutions
  • Build user-facing experiences that are intuitive, reliable, and deeply integrated into clinical workflows
  • Develop AI-enhanced operational tools that reduce administrative burden and improve care team effectiveness
  • Translate ambiguous operational challenges into scalable technical solutions
  • Iterate quickly based on user feedback and operational outcomes
  • Collaborate with Data and AI Platform teams to shape reusable platform primitives and workflow capabilities
  • Contribute to the evolution of our internal tooling ecosystem and operational technology strategy
  • Help establish best practices for rapidly building and deploying workflow applications in a healthcare environment

ABOUT YOU

  • You are a high-agency builder who enjoys owning problems end-to-end
  • You are energized by working closely with users and embedding deeply within operational workflows
  • You have strong product intuition and care about building polished, usable software
  • You thrive in fast-paced startup environments with evolving priorities and ambiguity
  • You are comfortable balancing speed, pragmatism, and technical quality
  • You enjoy collaborating across technical and non-technical teams
  • You are excited about applying AI capabilities to real-world operational challenges
  • You care about building systems that meaningfully improve how healthcare is delivered

From a cultural perspective, you are:

  • Accountable. You hold a high standard and you expect it from others too.
  • Someone who treats mistakes as data. You move fast, name what isn't working, fix it, and carry the lesson forward.
  • Customer-obsessed. You build genuine empathy with users, dig into what they actually need, and stay focused on solving real problems.
  • A problem-solver, not a finger-pointer. You own challenges alongside your teammates - no blame, no deflection, just shared accountability for finding the way forward.
  • A contributor who shapes the answer. You bring your strengths and perspective to every conversation because rigorous debate is how the strongest solutions get built.
  • Comfortable with uncertainty. The plan will change and the priorities will shift - you make smart calls with what you have and course-correct with good judgement when things evolve.

ADDITIONAL QUALIFICATIONS

  • 3+ years of experience building customer-facing or operational software applications
  • Background in software engineering, computer science, or a related technical field
  • Experience building full-stack applications or workflow tooling in modern development environments
  • Familiarity with APIs, data platforms and models, and integrating across multiple systems
  • Ability to independently design and implement technical solutions with limited oversight
  • Strong written and verbal communication skills
  • Experience working in high-growth or fast-moving environments preferred
  • Experience in healthcare, care delivery, or other operationally intensive service industries is a plus
  • Familiarity with AI-enabled applications, LLM workflows, or agentic systems is a plus
  • You live in Central (CT) or Eastern Time (EST) Zones

In accordance with the Illinois Pay Transparency Act, we're providing the full salary range for this position. Actual compensation will be determined based on location, relevant experience and qualifications. Most offers will fall within a portion of the listed range. • 401k plan with 4% company match + no vesting period
  • $720 - $1,000 added to employee Health Savings Account annually for eligible health plans
  • And more!

At Hopscotch Primary Care, we embrace diversity, invest in a culture of inclusion and positivity and encourage all to apply to join our team. You will receive consideration for employment without regard to race, color, religion, gender, gender identity or expression, sexual orientation, national origin, genetics, disability, age, or veteran status.
