Assumes overall responsibility for an assigned case load of home health patients, providing assessment, coordination of ancillary services and community services, and may provide direct nursing health to the patient in the patient's place of residence. May oversee and direct Licensed Practical Nurses (LPNs) Home Health Aides (HHAs) in their day-to-day assignments.
