As a Dual Adult Day Treatment & In Home Therapist at Sagent Behavioral Health, you will be providing services within the Adult Day Treatment program. This role will split time as an ADT Group Therapist and an individual Outpatient Therapist. The purpose of the program is to offer an intensive group psychotherapy experience for client to help reduce the effects of mental illness and provide skills training to empower clients to live in the community independently.

Responsibilities:
  • Required documentation includes timely completion of progress notes, individual treatment plans, functional assessments, LOCUS assessments, discharge plans, and discharge summaries under the supervision of a Mental Health Professional.
  • The role involves facilitating or co-facilitating ADT groups and providing crisis management services as needed.
  • Provide client oversight during breaks and follow up with clients who do not attend scheduled groups.
  • Consult with the ADT Supervisor when situations fall outside one's scope of competency and attend clinical staff meetings regularly.
  • Collaborate closely with other Adjunct Facilitators and communicate with outside providers regarding client progress or status.
  • Complete referrals to other Sagent programs, support care coordination efforts, and update the MN Mental Health Access website weekly.
  • Perform additional duties as assigned by the ADT Supervisor or ADT Program Director.

In Home Therapist Responsibilities:
  • Provide culturally responsive mental health skills to clients and/or their families in the home, community.
  • Assist clients in identifying goals for functional improvement and work with them to meet their goals.
  • Design and implement treatment/intervention in-home reflective of client needs.

Requirements:
  • Master's degree in one of the behavioral sciences or related fields from an accredited college or university.
  • Candidates will have clinical licensure or be on licensure track for
    • MN: LMFT, LPCC, LICSW, LP
  • Experience completing diagnostic assessments, treatment plans and clinical case notes with the ability to demonstrate and model stable, appropriate boundaries with clients.
  • Proficient in the use of Office 365 and EHR systems.

Compensation: This is an exempt position. Total earnings for this position will range from $55,000 - $68,000 annually.
