About Oakwood Creative Care

Oakwood Creative Care partners with older adults living with cognitive and physical challenges and the families who love them to help them live with joy, dignity, and connection at home. We specialize in dementia care, and our team delivers care coordination, caregiver education, and community resource navigation designed around one simple belief: growing older, or living with a cognitive or physical challenge, shouldn't mean giving up the things and people that make life worth living.

What You'll Do

  • Get to know the families you serve — Meet with families to understand what a good day looks like for them, and build a personalized plan that protects the moments, routines, and relationships they care about most.
  • Support caregivers — Provide dementia education, caregiver coaching, and practical strategies that ease stress and help caregivers rediscover moments of connection and joy with the person they're caring for.
  • Champion aging (and living) in place — Help patients stay in the home and community they love for as long as it's safe and possible, adapting support as cognitive or physical needs change over time.
  • Navigate resources — Connect patients and families to community resources — respite care, adult day programs, home health, support groups, legal and financial planning, and more -and follow through to make sure they actually get connected.
  • Stay connected — Check in regularly with patients and caregivers by phone, video, or in person, adjusting the frequency and focus of support as their needs change.
  • Respond with care — Be a calm, steady presence when a family is navigating a crisis, safety concern, or difficult transition, and know when to bring in additional support.
  • Collaborate — Work with the interdisciplinary team and community partners to make sure everyone involved in a patient's care is on the same page.
  • Support and Education- Facilitated education classes and support groups
  • Document your work — Keep clear, timely records of the support you provide using our electronic health record system.

What We're Looking For

  • A genuine belief that older adults -including those living with dementia or other cognitive and physical challenges deserve joy, dignity, and a say in how they live.
  • Bachelor's degree in Social Work (BSW) required; Master's degree (MSW) strongly preferred.
  • Current Arizona social work licensure (LMSW or LCSW), or actively working toward it.
  • 2+ years of experience working with older adults, people living with dementia, and/or family caregivers.
  • Familiarity with community resources for older adults in the Maricopa County area.
  • A warm, patient communication style - you're comfortable sitting with families through hard moments, and celebrating the good ones.
  • Comfortable with computers and electronic documentation systems.
