Responsibilities

• Perform occupational therapy evaluations and reassessments
• Develop individualized treatment plans
• Help patients improve safety, independence, and functional performance within their home environment
• Recommend adaptive equipment and environmental modifications
• Educate patients and caregivers
• Collaborate with physicians and interdisciplinary team members
• Complete documentation accurately and timely

Qualifications

• Active California Occupational Therapist License
• Valid driver's license
• Home health experience preferred, but not required.
