POSITION SUMMARY

The Patient Experience Manager (PEM) is responsible for guiding patients through their full care journey, ensuring a seamless, informed, and empowering experience from onboarding through ongoing treatment.

This role plays a critical part in delivering a consistent, high-quality patient experience by combining patient education, program coordination, and relationship management to support long-term engagement and outcomes.

The PEM ensures every patient feels confident, supported, and connected throughout their care journey.

Hours

Monday: 7:50am- 6pm

Tuesday: 6:50am-noon

Wednesday: 7:50am- 5pm

Thursday: 6:50am- noon

Friday: 6:50am- 2pm 

Sat and Sunday: OFF

PRIMARY RESPONSIBILITIES

Patient Journey Ownership

• Guide patients through onboarding, treatment progression, and ongoing care
• Educate patients on treatment options, timelines, and expectations
• Ensure patients remain engaged and consistent with their care plans
• Celebrate patient progress and reinforce long-term success

Patient Engagement & Communication

• Build strong, trust-based relationships with patients
• Address patient questions, concerns, and barriers to adherence
• Provide clear, simple, and proactive communication at every stage
• Ensure patients feel supported, informed, and confident in their care

Program Coordination & Support

• Manage patient accounts, subscriptions, and documentation
• Coordinate with clinical and administrative teams to ensure seamless care
• Support medication ordering, tracking, and follow-up
• Monitor patient activity and re-engage as needed

Clinic & Team Collaboration

This role requires flexibility, adaptability, and a team-first mindset. All team members are expected to support clinic operations and patient needs as they arise, within the scope of their role and licensure.

• Partner with PRCs and clinical staff to ensure a consistent patient experience
• Support clinic flow when needed to maintain efficiency and patient satisfaction
• Communicate effectively across teams to ensure alignment and continuity of care

This may include assisting with check-in, rooming patients, preparing rooms, supporting clinical staff, or helping maintain clinic flow-within the scope of their training and licensure.

Requirements

QUALIFICATIONS

• Strong communication and relationship-building skills
• Highly organized with attention to detail
• Ability to manage multiple patients and priorities simultaneously
• Proficiency in EMR, CRM systems, and Microsoft Office
• Patient-centered mindset with a passion for delivering exceptional experiences

ADDITIONAL INFORMATION

• This role supports both patient experience and clinic performance through engagement and coordination
• Cross-functional support may be required based on clinic needs
