Hospice Triage Nurse (Remote)

Are you a compassionate and experienced nurse dedicated to providing exceptional care to patients and their families during their most critical moments? If so, join our team at Legacy Hospice as a Triage Hospice Nurse and make a lasting impact on the lives of those in need!

Key Responsibilities:

  • Answer all incoming calls and communications and provide telehealth assessments, pain management, and symptoms.
  • Assess the patient and family's need for a home visit and scheduling through HCHB and notify the patient and family of the expected visit time.
  • Managing HCHB workflow including documentation of coordination notes and back office workflow.
  • Serves as an after-hours point of contact to ensure referrals are documented and ensure timely admissions.
  • Coordinate patient care and communication with interdisciplinary teams.

Key Qualifications:

  • Practical Nurse, Vocational Nurse, or Registered Nurse licensed in the state of employment

  • Certified Hospice and Palliative Nurse and/or ASN/BSN preferred

  • Minimum 3 years of hospice nursing experience

  • Remote access Wi-Fi and phone

  • Demonstrated expertise in pain management, symptom control, crisis intervention, and emotional and pre-bereavement support

  • Excellent critical thinking skills and clinical judgement

  • Previous experience of operating and documenting in Homecare Homebased

  • Excellent verbal and written communication skills

  • Self-directed and able to work with minimal direct supervision

  • Ability to work outside of posted regular business hours, including overnights and weekends
