Description:

The Community Support Services Team Support Assistant supports the management and day-to-day coordination of community programs. Primary responsibilities include assigning individuals to DSPs, managing staff schedules, tracking billing units for CSS DSPs, attending annual individual service plan meetings, and monitoring individual goals. This role may also assist with monthly performance reviews, interviewing, hiring, and training new CSS DSPs, as well as providing in-ratio, in-home, and community-based support as needed.

The Team Support Assistant helps ensure that program participants have access to meaningful, enriching activities in areas such as recreation, volunteerism, culture, religion, health, education, skill development, and vocational exploration. This role works closely with the Program Manager of Community Support Services and maintains documentation and program operations in accordance with CARF standards.

Key Responsibilities: (Descriptions represent common responsibilities for the position. Additional responsibilities may be added or removed as needed)


1. Schedule and Availability:

  • Work an exempt 40-hour schedule across varying days as needed
  • On-call availability for after-hours contact from staff, individuals, and/or caregivers

2. Supervisory Support:

  • Assist the Program Manager with scheduling, caseload coordination, and administrative tasks
  • Meet all required paperwork and documentation deadlines, including monthly reviews, census data, etc.
  • Assign CSS DSP staff to new and existing individuals receiving CSS services
  • Create, update, and track staff schedules
  • Track billable units for CSS individuals
  • Assist or conduct monthly meetings with staff
  • Maintain and appropriately use a company-issued cell phone and/or tablet for communication with staff, management, and supported individuals
  • Perform other duties as assigned during emergencies or as directed by leadership

3. Training and Development:

  • Provide and/or track training for new CSS DSPs

4. CSS DSP Staff Support:

  • Complete in-ratio hours when regular staff are unavailable
  • Assist with in-home and community-based activities as needed
  • Provide timely updates and address concerns related to interventions and support needs

5. Documentation and Reporting:

  • Maintain accurate records of staff performance, client progress, and program metrics
  • Prepare timely reports for internal and external stakeholders

6. Client Advocacy:

  • Attend and assist with annual planning meetings for individuals served
  • Assist with IP goal tracking and follow-up
  • Promote client-centered approaches that preserve dignity and individual preferences
  • Maintain positive relationships with supported individuals, families, other agencies, and staff
Requirements:

Education:

  • High school diploma or GED

Required Training and Certifications

  • First Aid & CPR
  • Crisis Prevention and Intervention (CPI)
  • Universal Precautions
  • Lifting Techniques
  • Medication Administration (CMA/L1MA)
  • Knowledge of relevant state and federal regulations
  • Annual renewal of all certifications is required

Skills and Abilities:

  • Strong leadership and team management capabilities
  • Excellent verbal and written communication skills
  • Excellent organizational skills with sharp attention to detail
  • Proficiency with computers and common office software
  • Demonstrated maturity, flexibility, sound judgment, time management, and interpersonal skills

Essential Physical Requirements:
