Symbii Home Health and Hospice is seeking a RN Case Manager to join our amazing team!

  • Provide the patient and their caregivers counseling and education to facilitate overall wellness.
  • Communicate with the physician, your interdisciplinary team, and other caregivers to coordinate the best care possible for your patients.
  • Prepare clinical notes and identify discharge planning needs as part of the care plan development and implement prior to discharge of patient.
  • Instruct, supervise, and evaluate CNAs and LPNs, as directed.
Job Requirements
  • Must possess a valid RN license in the State of Idaho
  • Must hold a current CPR certification
  • Must hold a valid driver’s license and current auto insurance
