We are seeking a Full-Time LPN Audit Nurse in our Agency!

Responsibilities & Duties for Full-Time LPN Audit Nurse:

Develop initial consumer ISP's (upon admission) and maintain as appropriate (annually and with significant changes).

Ensure ISP's and UAI's are uploaded and available in Credible

Ensure UAI's are up-to-date and matches the ISP at all times.

Periodic consumer chart audits

Periodic medication cart audits

  • Maintain accurate consumer records and monitor for continuity of care.
  • Assist Nure Managers and RN Supervisor with PCP/Doctor visits with consumers as needed
  • Administer prescribed medications as needed.
  • Educate consumers on medication compliance and potential side effects.
  • Assess vital signs, height, weight, and monitor medication compliance.
  • Collaborate with external vendors to ensure quality care is provided (Hospice, HHC, OT/PT, Labcorp, Family Care Pharmacy, etc.
  • Ensure the monthly POS's are accurate and all DSS required information is maintained
  • Ensure all required documentation is in QMAR
  • Run Audit reports upon request
  • Complete prior authorizations and assist with psychiatric medication needs as needed
  • Ensure compliance with agency policies and relevant local, state, and federal regulations.

Qualifications and Skills for Full-Time LPN:

  • Graduation from an accredited Licensed Practical Nurse Program.
  • Current, unrestricted LPN license in Virginia or from a Nurse Licensure Compact State.
  • Minimum of 1 year of LPN experience in a mental health program and/or residential setting.
  • Knowledge of Department of Social Services ALF regulations
  • Minimum of 1 year of LPN experience working with individuals with intellectual disabilities.
  • Valid driver's license with an acceptable DMV record.
  • Proficiency in Microsoft Office.
  • Preferred: Experience with electronic medical records and basic knowledge of Medicaid/Medicare regulations.
