Job Description
The Patient Education Programs Educator will be instrumental in helping to drive volume of CKD participants educated to the Kidney Smart program by conducting direct education virtually, support with core operations for the program and provide increased capacity for our field teammates across the country to increase the number of participants receiving life changing CKD education through Kidney Smart.
ESSENTIAL DUTIES & RESPONSIBILITIES
1. Provide telephonic call support to CKD participants in focused areas across the country including education, scheduling, rescheduling, and targeted Spanish support.
2. Review, educate and answer questions on all modality options for dialysis including transplant, hemodialysis, PD and HHD.
3. Provide local coverage support with opportunities and data entry as needed. 4. Complete data entry & tasking into data management system.
