Position Overview

Fort Behavioral Person-centered Therapy is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our remote team. Work Environment

This position offers a unique remote work setting that provides flexibility while ensuring access to high-quality care for our clients. The work hours are primarily Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend hours required based on client needs. You will have the flexibility to create a schedule that accommodates client visits and ensures continuous care, along with potential on-call responsibilities for urgent situations.

Key Responsibilities

• Conduct comprehensive psychosocial assessments of senior clients, taking into account their emotional, mental, and social well-being.
• Develop and implement personalized care plans in collaboration with clients and their families, addressing unique goals and preferences that enhance their quality of life.
• Provide emotional support and counseling to clients, helping them navigate significant life transitions and tackle various challenges they face.
• Act as an advocate for clients’ rights, ensuring they have access to necessary community resources and remedial services.
• Collaborate with interdisciplinary teams, including healthcare professionals and caregivers, to coordinate care effectively and enhance overall service delivery.
• Conduct regular check-ins with clients to monitor their well-being, living conditions, and address any safety concerns.
• Facilitate communication among families involved in care, navigating conflicts and enhancing collaboration around the client’s needs.
• Educate clients and their families about available community resources, support groups, and programs that can positively affect their social engagement and mental health.
• Maintain accurate and up-to-date records of client assessments, care plans, and progress notes to ensure quality of care and compliance with regulations.
• Stay current on social work best practices, policies, and ethical standards to provide informed and effective client support.

Patient Population / Client Focus

The primary focus of this role will be seniors and their families who face a range of mental health challenges. You will engage with a diverse array of clients, each with unique stories and needs, including but not limited to issues related to aging, anxiety, depression, and transitions in life, such as moving to assisted living or coping with loss. Your work will directly contribute to improving their quality of life and restoring their sense of agency and emotional health.

Qualifications

• A Master’s degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of experience in a social work role, with preference given to those who have worked in geriatric or community center settings.
• Comprehensive knowledge of social work principles and aging-related issues, along with an understanding of community resources available to support clients.
• Exceptional communication skills and the ability to build rapport with clients and families.

Preferred Experience

Experience in interdisciplinary teams, especially within healthcare settings, is highly valued. Familiarity with case management software and electronic health records is a plus, as is knowledge of local community resources for seniors and mental health services. A proactive approach to problem-solving and a deep commitment to advocating for client needs will help you excel in this position.

Compensation and Benefits

We offer a competitive salary ranging from USD 70,000 to 100,000 per year, commensurate with experience and qualifications.
