Responsibilities
  • Deliver direct patient care in accordance with care plans and physician orders

  • Supervise, guide, and support CNAs to ensure high-quality care

  • Administer medications and treatments as prescribed

  • Monitor residents’ conditions and report changes promptly

  • Coordinate resident admissions, discharges, and transfers

  • Maintain accurate documentation in PointClickCare (PCC)

  • Foster resident-centered care and build strong relationships with families

Qualifications
  • Active, unrestricted Illinois RN or LPN license

  • Current CPR certification

  • Strong communication and teamwork skills

  • Ability to provide compassionate, quality care in a fast-paced environment
