• Conduct in-depth psychosocial assessments to gather critical information concerning the mental health, strengths, and circumstances of hospice patients and their families.
• Provide therapeutic support through individual, family, and group therapy sessions, utilizing evidence-based treatment modalities such as cognitive-behavioral therapy and solution-focused approaches.
• Develop and implement personalized treatment plans with clear goals and interventions to enhance the emotional and psychological well-being of our clients.
• Work collaboratively with a multidisciplinary team, including psychiatrists and psychologists, to ensure coordinated and comprehensive care that addresses all facets of the patient's experience.
• Offer crisis management, risk assessments, and safety planning to ensure the stability and support of clients during vulnerable moments.
• Maintain thorough and accurate documentation of patient interactions and treatment plans, complying with ethical and legal standards.
• Facilitate group therapy sessions, psychoeducation classes, and support groups that empower clients and their families.

This role centers around hospice patients facing the complexities of terminal illness and end-of-life transitions. You will provide empathetic and culturally sensitive care to individuals and families, addressing not only mental health needs but also emotional support during a significant life phase. Understanding and navigating the grief process is critical as you assist clients in finding peace and resilience during challenging times.

• Master's degree in Social Work from an accredited program, emphasizing clinical pathways suitable for effective patient care.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Georgia.
• A minimum of two years of hands-on experience in a mental health or outpatient setting, with a preference for hospice or end-of-life care exposure.
• Proficient knowledge of therapeutic modalities and experienced in utilizing evidence-based practices to inform treatment approaches.
• Exceptional interpersonal and communication skills, ensuring a compassionate, client-centered approach in all interactions.

• Experience working with hospice or palliative care populations is advantageous.
• Familiarity with trauma-informed care practices and techniques for managing grief and bereavement.
