About the Role

Clarvida is seeking an ACT Nurse (RN) to join our Assertive Community Treatment (ACT) Team serving adults with severe and persistent mental illness. In this role, you will be a critical part of a multidisciplinary team providing intensive, community-based services designed to support recovery, stability, and independence.

ACT services are delivered where clients live, work, and spend time in the community. You will support a specialized caseload, helping clients achieve treatment plan goals while remaining integrated with family, friends, and community supports. This position includes use of a company vehicle for client transportation as needed and participation in a rotating on-call schedule, which provides additional earned PTO.

Responsibilities

Provide nursing services as part of the ACT multidisciplinary team
Meet with clients in homes, community settings
Conduct basic health assessments, including vitals and labs
Support medication management and facilitate long-acting injectable medications
Coordinate care with medical providers and psychiatric staff
Support clients with severe and persistent mental illness and/or substance use disorders
Participate in a rotating on-call schedule (approximately one week every 6-8 weeks)
Provide client transportation using a company vehicle as needed
Maintain accurate, timely, and compliant documentation

Requirements

Active Registered Nurse (RN) in the State of Louisiana
Experience working with individuals with severe and persistent mental illness and/or substance use disorders
Psychiatric nursing experience preferred
Valid Driver's License in good standing and current automobile insurance
