Registered Nurse (RN) – Home Health | Per Diem

Airmid Home Health Care is seeking experienced and compassionate Registered Nurses (RNs) to provide skilled nursing care to patients in their homes.

Pay: $50–$140 per visit

Job Type: Per Diem

Work Setting: Field-Based / Patient Homes

Responsibilities

Provide skilled nursing visits and comprehensive patient assessments.

Perform wound care, dressing changes, IV therapy, and other skilled nursing services as ordered.

Develop, implement, and evaluate individualized plans of care.

Monitor patient conditions and report significant changes to physicians and the clinical team.

Coordinate care with physicians, Clinical Managers, PT, OT, ST, MSW, and HHA staff.

Complete OASIS, Start of Care, Recertification, Resumption of Care, Transfer, and Discharge documentation as required.

Maintain accurate and timely documentation through Synergy Software.

Provide patient and caregiver education.

Perform Home Health Aide supervisory visits as required.

Follow all applicable state, federal, Medicare, and agency regulations.

Requirements

Current, unrestricted California RN License.

BLS certification through AHA or American Red Cross.

IV/Phlebotomy certification.

NSO or equivalent professional liability insurance.

Minimum 1 year of nursing experience.

Minimum 1 year of Home Health/Visiting RN experience.

Strong clinical assessment, communication, and documentation skills.

Ability to work independently and manage a patient caseload.

Valid driver's license, reliable vehicle, and required auto insurance.

Preferred: OASIS experience and 2+ years of nursing experience.
