Activa Home Health, a licensed, Medicare-certified agency, is seeking skilled and compassionate Registered Nurses to provide Medicare home health visits in both full-time and per-diem roles. Key Responsibilities

  • Deliver skilled nursing care per the Plan of Care during Medicare home health visits
  • Accurately document visits in the EMR, including OASIS assessments
  • Communicate changes in patient condition promptly with the interdisciplinary care team
  • Provide clinical services within scope of practice, including:

    • Medication administration
    • Wound care
    • Infusion therapy
    • Catheter care
    • Safe transferring and positioning of patients


  • Collaborate with Nurse Supervisors and Managers of Clinical Practice
