with compensated on-call rotation

As an RN Clinical Coordinator at LifeCircles PACE, you'll provide patient-centered nursing care across clinic, home, and facility settings while enjoying a schedule that promotes work-life balance. Partnering with an Interdisciplinary Team, you'll provide holistic, high-quality care that improves outcomes and helps older adults live with dignity, independence, and confidence.
  • Deliver Direct Nursing Care - Assess acute needs, report changes in condition to the clinical team, and complete skilled nursing tasks including wound care.
  • Conduct Health Assessments and Adjust Care Plans - Complete periodic health assessments and participate in interdisciplinary care planning.
  • Coordinate Interdisciplinary Care - Participate in daily interdisciplinary team meetings and work with teams to enhance participants' quality of life.
  • Provide Case Management- Coordinate care with participants, families, and outside agencies to improve outcomes.
  • Engage in Ongoing Education - Maintain certifications and stay current with best practices in an environment that promotes ongoing learning.

Education
