Description

PromptCare is seeking a highly motivated and experienced nurse (RN or LPN) to join our team as a Clinical Nurse Liaison Educator (CNL). This role serves as both a clinical educator and liaison, providing exceptional support to referral sources, patients, and caregivers. The CNL delivers comprehensive education and hands-on training to ensure safe, effective home infusion therapy and successful transitions of care from hospital to home.

The ideal candidate has strong infusion experience, excels in patient education, and is passionate about delivering a high-touch, "white glove" service experience.


Job Type: Full-time 

Schedule: Monday through Friday  and on call every 5th week but is subject to change 

Location:  Northshore Louisiana location (Covington, LA and Hammond, LA)

Requirements

  • Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of employment, in good standing
  • Willingness to obtain additional state licensure (company-sponsored), if applicable
  • Valid driver's license and active automobile insurance
  • Minimum of 5 years of nursing experience performing infusion therapy in a hospital, outpatient, or clinical setting
  • Experience should include administration of IV antibiotics, IV fluids, and/or similar therapies
  • Home infusion experience preferred but not required
  • Experience working with hospital systems, discharge planning, or physician referrals preferred
  • Strong computer skills (EMR documentation, email, electronic charting)
  • Excellent communication, critical thinking, and patient education skills
  • Ability to work independently in a field-based setting
  • Demonstrated commitment to patient care, service excellence, and business growthKey

Responsibilities:

  • Provide thorough patient and caregiver education on disease processes, treatment plans, medications, infusion therapies, and equipment use.
  • Assess patients' and caregivers' learning needs, preferred learning styles, and readiness to learn to ensure education is effective and accessible.
  • Deliver hands-on training and demonstrations, including medication administration, device operation, safety procedures, and symptom management.
  • Reinforce understanding through teach-back methods and provide ongoing support to confirm safe and competent care at home.
  • Develop and update patient education materials in compliance with clinical guidelines and organizational standards.
  • Serve as a clinical resource to the interdisciplinary care team regarding patient progress, challenges, or additional educational needs.
  • Document all education provided and communicate any concerns or barriers to the care team to ensure continuity of care.
  • Promote patient confidence, independence, and adherence through compassionate guidance and evidence-based teaching.
  • Evaluates referral source and patient experiences for opportunities to improve customer satisfaction
  • The CNL collaborates with referral sources, the patients/family members and PromptCare's Intake and Nursing teams to facilitate patient discharge and coordination of care
  • Teaching the patients/family members about their therapy and supplies and performing the initial hook-up of patients to their IV therapy to facilitate discharge to the home are also critical elements of the job
  • Provides the referral sources, physicians, PHI salespeople and patients/family members with timely and accurate information and takes appropriate corrective actions regarding our services to ensure complete satisfaction
  • Ability to properly triage problems in the field in accordance with company standards and other verbal and written training policies
  • Follow proper procedures for accurate and timely documentation of referral sources or patient concerns, and most importantly, take appropriate corrective actions
  • Maintains strong knowledge of PromptCare's Standard Operating Procedures (SOPs) and ensures that they are applied consistently
  • Maintains compliance with all applicable laws, regulations, and SOPs at all times
  • Conducts activities with a mindset of delivering "white glove" service to the referral sources and patients/family members
  • Other duties as assigned
