About the Role:

The Individualized Skills & Socialization (ISS) Provider plays a critical role in delivering high-quality Supervision Services to individuals requiring structured support in community integration through social outings and on-site service that focus on acquiring and retaining essential independent living, communication and adaptive skills within the structured center located in Pearsall, Texas.   This position focuses on ensuring the safety, well-being, and progress of individuals by implementing individualized care plans and monitoring compliance with established protocols. The ISS Provider collaborates closely with multidisciplinary teams, including the individual, their family, and service coordinators, to facilitate holistic care and positive outcomes. Through consistent observation, documentation, and intervention, the provider helps individuals develop essential life skills and maintain community integration. Ultimately, this role contributes to enhancing individuals quality of life while supporting organizational goals for effective service delivery.

Minimum Qualifications:

  • High school diploma or equivalent required; associate degree or higher in a related field preferred.
  • Ability to pass background checks and any required health screenings.
  • Basic computer usage.
  • Able to drive a wheelchair van to transport individuals.

Preferred Qualifications:

  • Lives within the service are of Frio and LaSalle Counties.
  • Experience working with individuals diagnosed with Intellectual and Developmental Disabilities.

Responsibilities:

  • Provide direct supervision and support to clients in accordance with individualized service plans and regulatory guidelines.
  • Monitor client behavior and progress, documenting observations and reporting any significant changes or concerns to the care team.
  • Implement therapeutic interventions and assist clients in developing coping strategies and life skills.
  • Coordinate with healthcare professionals, individual, and family members to ensure continuity of care and address client needs.
  • Maintain compliance with all organizational policies, safety protocols, and legal requirements related to client care and confidentiality.
