What is an Island RN?

An Island RN is a Registered Nurse who works on an island, often in remote or rural healthcare settings. These nurses provide comprehensive care to residents and visitors, frequently managing a wide range of medical needs due to limited local resources. Island RNs may work independently or as part of a small team, handling emergency care, routine check-ups, and patient education. They play a critical role in ensuring the health and safety of island communities, often adapting to unique challenges such as transportation delays or supply shortages.

What skills and qualifications are needed to thrive as an Island RN?

To thrive as an Island RN, you need strong clinical and emergency care skills, a nursing degree, active RN licensure, and experience working independently in remote or rural settings. Familiarity with telemedicine systems, emergency medical equipment, and electronic health records is often required. Excellent problem-solving, adaptability, and communication skills are crucial for handling diverse patient needs and limited resources. These abilities ensure effective patient care, quick decision-making, and optimal health outcomes in isolated healthcare environments.

What unique challenges do nurses face when working on an island, and how can they prepare for them?

Island RNs often encounter challenges such as limited access to specialized medical resources, the need to handle a wider variety of cases, and occasional isolation from larger healthcare networks. To prepare, it's important to develop strong clinical assessment skills, adaptability, and the ability to work independently or in small teams. Building solid relationships with local healthcare providers and staying updated through remote continuing education can also help overcome these challenges and ensure high-quality patient care.

Position Overview
Are you an ambitious Nurse Practitioner or Physician Assistant with a passion for hands-on, procedure-focused care? Our premier interventional pain management practice is seeking a dynamic, procedurally minded Advanced Practice Provider (NP/PA) to join our multidisciplinary team.
This is not a passive medication management role. You will function at the top of your license, focusing heavily on advanced diagnostics, pre- and post-procedural care, and performing autonomous, minimally invasive, image-guided interventions to restore function and improve our patients' quality of life. We provide a collaborative, state-of-the-art environment with built-in mentorship and cutting-edge fluoroscopic and ultrasound technology.

Core Procedural Responsibilities
  • Perform Autonomous Interventions: Conduct low-to-moderate acuity interventional procedures independently or under supervision, including trigger point injections (TPI), large and small joint injections, bursa injections, and peripheral nerve blocks.
  • Utilize Advanced Imaging: Operate and interpret ultrasound-guided technology for precise point-of-care needle placement and diagnostic accuracy.
  • Surgical & Advanced Procedure Assistance: Assist interventional physicians in complex fluoroscopic cases, including epidural steroid injections, radiofrequency ablations (RFA), and spinal cord stimulator (SCS) trials/implants.
  • Neuromodulation Management: Conduct patient selection, pre-operative planning, and post-operative programming for spinal cord stimulators and intrathecal pumps.
  • Comprehensive Diagnostics: Conduct targeted orthopedic and neurological physical exams, ordering and interpreting advanced imaging (MRIs, CT scans, X-rays) to determine precise procedural targets.

Clinical Responsibilities
  • Patient Optimization: Manage pre-operative evaluations to medically clear and educationally prepare patients for upcoming spinal and joint interventions.
  • Continuum of Care: Oversee post-procedure follow-ups to evaluate treatment efficacy, manage recovery, and adjust functional rehabilitation plans.
  • Multimodal Treatment Planning: Develop comprehensive care paths integrating physical therapy, regenerative medicine, and non-opioid pharmacological strategies.
  • Compliance & Advocacy: Maintain precise electronic medical record (EMR) documentation in accordance with strict state and DEA regulatory guidelines.

Position Highlights & Benefits
  • Procedural Mentorship: Dedicated on-site training program for ultrasound guidance and interventional techniques.
  • Schedule: Balanced Monday–Friday outpatient schedule with no nights, no weekends, and no hospital call.
  • Compensation: Highly competitive base salary ranging from $165,00+ (depending on experience) plus a productivity bonus structure.
  • Comprehensive Package: Full medical/dental/vision coverage, matching 401(k), generous PTO, and an annual CME stipend for procedural courses.

Qualifications & Skills

    Physician Assistant (PA-C).
  • Credentials: Valid DEA registration with controlled substance prescriptive authority.
  • Experience: Minimum of 1 year of experience in Interventional Pain, Orthopedics, Neurology, or PM&R preferred, but highly motivated new graduates with strong surgical/procedural rotations will be considered.
  • Traits: Exceptional manual dexterity, visual-spatial awareness for needle placement, and a compassionate bedside manner.
