Position Summary:
The RN – Resident Care Manager is responsible for overseeing clinical operations within our assisted living community. You will serve as the primary clinical liaison among residents, families, healthcare providers and our care staff. You will ensure regulatory compliance, champion person-centered care, and lead the resident care team to deliver exceptional outcomes.

Essential Duties and Responsibilities:

Clinical & Care Coordination:

  • Develop, implement, modify and communicate resident assessments and care plans.

  • Coordinate resident assessments, care plans and ongoing evaluations in compliance with state licensing requirements.

  • Supervise and support the care team (Med Aides, CNAs, Personal Care Assistants) to ensure consistent and compassionate care.

  • Conduct initial and periodic resident assessments, medication reviews, and service-plan updates.

  • Monitor residents’ health conditions and promptly communicate changes to physicians, families, and leadership.

  • Coordinate care with outside agencies such as home health, hospice, and therapy providers.

  • Participate in prospective resident move-in assessments and resident/family care conferences.

  • Ensure changes in resident conditions are evaluated, reported, monitored, documented and followed up.

Operations & Regulatory Compliance:

  • Maintain accurate, timely and compliant resident records and documentation.

  • Oversee medication-management systems: ensure proper storage, administration and disposal in accordance with physician orders and regulations.

  • Partner with the Executive Director to ensure appropriate staffing, training and scheduling within the care department.

  • Create and maintain monthly care-staff schedule in the electronic scheduling platform, ensuring compliance with staffing ratios, state rules and budget.

    • Arrange replacement staff when necessary; may perform duties as caregiver or medication aide to meet staffing ratios.

  • Conduct resident rounds regularly to monitor and evaluate quality and appropriateness of care.

  • Participate in QA audits, infection-control checks, incident follow-up and support state/federal survey processes.

  • Pull compliance reports (e.g., MatrixCare/Yardi) daily and follow up on non-compliance areas.

  • Maintain current knowledge of OSHA and state regulations and monitor community compliance.

  • Attend daily stand-up meetings with leadership team and conduct daily stand-up with care staff to relay pertinent resident-care and wellbeing information.

  • Provide direct supervision of resident-care staff: monitor job performance, collaborate on evaluations, coach and counsel staff, document corrective actions as needed.

  • Serve as a member of the community’s leadership team and the QA Committee.

  • Perform other duties as assigned by the Executive Director or upper management.

Qualifications:

  • Education:

    • Graduate of an accredited school of nursing (2-year or 4-year program).

    • Current Registered Nurse (RN) license in good standing with the State Board of Nursing.

  • Experience & Skills: 

    • 2-3 years of experience working with senior adults in skilled nursing or assisted living is preferred.

    • Ability to communicate effectively with residents, families, staff, community/state officials and the general public.

    • Exceptional organizational skills and ability to re-prioritise daily tasks to accommodate fluctuating needs of residents and community.

    • Strong documentation skills and attention to detail.

    • Technology proficiency—able to learn and master software programs for high job performance.
      Valid driver’s license.

    • Ability to work on-call and/or cover call-outs if coverage is not secured.

    • Must pass a criminal background check and meet all health requirements.

    • Ability to perform essential job functions with or without reasonable accommodation.

  • Physical Requirements:

  • Works a varied schedule; may include quarterly overnight shifts.

  • Medium work: ability to exert 20-50 pounds of force occasionally, and/or 10-25 pounds frequently, and/or up to 10 pounds constantly to lift, carry, push, pull or otherwise move objects.

  • OSHA Category II: potential exposure to bloodborne pathogens.

  • Involves interaction with personnel and visitors under all conditions and circumstances.
