Responsibilities
  • Conduct comprehensive patient assessments
  • Develop and implement individualized care plans
  • Administer medications, treatments, and nursing interventions

  • Provide skilled nursing services, including:

    • IV Therapy
    • Catheter Care and Management
  • Educate patients and families on:

  • Ensure compliance with all state and federal regulations
  • Provide compassionate care and emotional support to patients and families
Qualifications
  • Active New York State Registered Nurse (RN) License (Required)
  • CPR/BLS Certification (Required)
  • Minimum 1โ€“2 years of nursing experience preferred
  • Home Care, Geriatric, Hospice, or Related Clinical Experience Preferred
  • Strong clinical assessment and critical thinking skills
  • Ability to work independently while maintaining communication with the clinical team
  • Excellent interpersonal and communication skills
  • Experience with Electronic Health Records (EHR) preferred
  • Valid Driver's License and Reliable Transportation Preferred
