About the Role:Coney Island Homecare is looking for a compassionate and dedicated Hospice RN Case Manager to join our team in Bozeman, MT. In this meaningful role, you'll provide expert clinical care and emotional support to patients and families during life's most important moments. Responsibilities:- Manage a caseload of hospice patients, conducting comprehensive assessments and developing individualized care plans
- Perform skilled nursing visits in patients' homes, monitoring and documenting changes in condition
- Coordinate interdisciplinary team (IDT) care including physicians, social workers, chaplains, and aides
- Educate patients and families on disease progression, comfort measures, and end-of-life care
- Provide on-call support and respond to urgent patient needs as required
- Ensure compliance with Medicare/Medicaid hospice regulations and documentation standards
- Communicate effectively with physicians regarding patient status, symptom management, and medication changes
Requirements:- Active RN license in the state of Montana (or compact license eligibility)
- Minimum 1-2 years of nursing experience; hospice, palliative care, or home health experience preferred
- Strong knowledge of end-of-life care, pain management, and symptom control
- Excellent clinical assessment, documentation, and case management skills
- Valid driver's license and reliable transportation for home visits in the Bozeman area
- Proficiency with electronic medical records (EMR) systems
- Empathetic, compassionate communicator with strong family support skills
