What Youโ€™ll Be Doing:

You will be a welcomed member of the Long Term Care team at Fieldstone commons Community Community, working in a fast-paced environment where you will make a meaningful difference.ย  ย As the Associate Director of Care, your responsibilities will include overseeing clinical operations, leading clinical staff, and ensuring regulatory compliance to enhance the health, safety and well being of residentsย 

Clinical Managementย 

โ€ข Provide clinical leadership and guidance to nursing staff to ensure the delivery of safe, effective and person-centered care to residentsย 

โ€ข Collaborate with healthcare professionals to develop and implement care plans that address the individual needs and preferences of residentsย 

โ€ข Monitor and evaluate the quality of care provided, identifying areas of improvement and implementing evidence-based practices to enhance resident outcomesย 

Team Member Supervision and Development

โ€ข Train, supervise and evaluate nursing professionals, including Registered Nurses (RN), Registered Practical Nurses or Licenced Practical Nursesย  (RPN/LPN) and Personal Support Workers or Health Care Aides (PSW/HCA)

โ€ข Foster a positive work environment that promotes teamwork, communication and professional growth among team membersย 

โ€ข Provide ongoing education and training to nursing professionals on clinical best practices, regulatory requirements and resident centered care approachesย 

Regulatory Complianceย 

โ€ข Ensure compliance with all relevant legislation, regulations and standards governing long-term care homes in Ontario or British Columbiaย 

โ€ข Maintain accurate documentation and records related to resident care, medication administration and incident reporting to meet regulatory requirements and accreditation standardsย 

โ€ข Coordinate and participate in regulatory inspections, audits and surveys addressing any deficiencies and implementing corrective actions as necessaryย 

Quality Assurance and Continuous Improvementย 

โ€ข Implement quality assurance measures to monitor and evaluate the quality of care and services provided to residentsย 

โ€ข Conduct regular reviews of clinical policies, procedures and protocols to ensure alignment with best practices and regulatory requirementsย 

โ€ข Collaborate with interdisciplinary teams to identify opportunities for quality improvement and implement initiatives to enhance resident care outcomesย 

Interdisciplinary Collaborationย 

โ€ข Collaborate with other members of the interdisciplinary care team, including physicians, therapists, social workers and dietary staff to coordinate care and support the holistic needs of residentsย 

โ€ข Participate in care planning meetings, case conferences and family meetings to discuss resident care needs and preferences and develop comprehensive care plansย ย 

Who You Are:

โ€ข You are compassionate and dedicated to meaningful work that supports seniors in living life fullyย 

โ€ข You have completed a Bachelor of Nursing Science or have an equivalent post diploma education

โ€ข You are currently registered and in good standing with the College of Nurses

โ€ข Minimum of 2 yearsโ€™ experience in infection control is required

โ€ข Associate โ€“ Infection Prevention & Control (a-IPC) and/or Certification in Infection Control (CIC)

โ€ข Have minimum 1 year experience working as a nurse in Long Term Care

โ€ข Have a minimum of 2-3 years experience working as a Registered Nurse/Registered Practical Nurse in a managerial or supervisory capacity in a healthcare setting
