Job description

Position Summary

We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Assertive Community Treatment (ACT) Team. This position plays a vital role in providing comprehensive nursing care and care coordination for individuals with mental illness and substance use disorders. The RN will work as part of an interdisciplinary team to assess client needs, coordinate services, monitor medications, provide patient education, and support clients in achieving improved health and wellness outcomes within the community.

Key Responsibilities

Clinical Care & Assessment

  • Conduct comprehensive assessments of clients' physical, psychiatric, social, and medical needs.
  • Monitor clients for medication effectiveness, side effects, and adherence concerns.
  • Provide long-acting psychotropic medications via intramuscular (IM) injections in community settings, including correctional facilities when applicable.
  • Perform medication reconciliation and coordinate medication delivery and refill management.
  • Monitor and triage mental health symptoms, medication side effects, and physical health concerns.
  • Review laboratory results and coordinate appropriate follow-up care.
