In this pivotal role, you will provide therapeutic services to a diverse clientele, including adults, seniors, children, and teenagers, while collaborating closely with interdisciplinary teams. You will operate from the comfort of your own home, connected to a supportive team of healthcare professionals. While you will primarily engage with clients through virtual sessions, our office-based outpatient resources are just a call away, ensuring that all necessary support services are easily accessible.

• Provide individual and group clinical supervision to Master of Social Work (MSW) students and interns, fostering their professional development.
• Maintain accurate electronic medical records in accordance with departmental and health system protocols, ensuring compliance with regulatory standards.
• Conduct comprehensive psychosocial assessments of patients and families to identify their social, emotional, and physical needs, utilizing advanced social work theories and techniques.
• Collaborate with interdisciplinary healthcare teams to develop, implement, and monitor individualized psychosocial treatment plans that address the unique challenges faced by patients and their families.
• Engage in case finding for patients and families experiencing psychosocial needs, participating actively in team conferences and interdisciplinary rounds as necessary.
• Provide supportive counseling and crisis intervention services to help clients navigate the complexities of illness, adapt to medical information, and manage transitions in care.
• Facilitate psychoeducation sessions aimed at helping patients and families understand the implications of diagnoses, treatment options, and the journey from youth to adult services.
• Contribute to quality assurance efforts by participating in community improvement initiatives and assisting in the development of policies and procedures relevant to social work practices.
• As needed, make mental health diagnoses to inform subsequent treatment planning.

The focus of our services encompasses a wide array of patient populations, including adults, seniors, children, and adolescents. You will address a variety of mental health issues, from anxiety and depression to complex family dynamics and grief. The diverse needs of your clientele will demand versatility and a commitment to culturally competent care, ensuring that all individuals feel understood and supported.

• A Master’s degree in social work, psychology, or a related behavioral science field from an accredited institution is mandatory.
• Active licensure as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC).
• Demonstrated proficiency in assessment, interviewing, and counseling techniques, with the ability to synthesize complex information into comprehensive case narratives.
• Solid understanding of social work theories, principles, and techniques, applied effectively in clinical practice.

• Two or more years of clinical experience in a mental health setting, particularly in outpatient care.
• Experience working with diverse populations and tailoring interventions to meet unique cultural and individual needs.
• Proven track record of working collaboratively within interdisciplinary teams to achieve patient-centric outcomes.
