Job description

The Social Services Specialist responsibilities include:
  • Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community
  • The Social Service Specialist II provides therapeutic interventions through assessment, coordination, referral and interdisciplinary planning
  • They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs
  • Possess a clear understanding of discharge and transition planning and linkage with community resources
  • Demonstrates skills in planning, organizing, and managing multiple functions and complex processes

Position details:
  • Location: Mease Countryside Hospital - Safety Harbor, FL
  • Status: Part time, 16 hours per week
  • Schedule: Saturday & Sunday 8:00 AM - 4:30 PM
  • Weekend requirement: Every
  • On Call: No

Certifications and Licensures:
  • Preferred ACM (Case Management) or
  • Preferred CCM (Case Manager)

Education:
  • Required Masters in Social Work, Sociology, or Related Field
  • Strongly Preferred Masters Degree in Social Work

Experience:
  • Required 6 months Internship or
  • Required 1 year Social Work in an acute care or hospital setting
