Overview
The Diabetes RN/RD, CDCES in conjunction with multidisciplinary team members of the UF Health Pediatric Endocrinology outpatient diabetes clinic, is responsible for providing ongoing compassionate care for patients and families affected by diabetes. The Diabetes RN/RD: �Conducts assessment of each patient�s/family�s baseline knowledge of and adherence to their diabetes management regimen to determine their educational needs required to manage diabetes, and provides appropriate ongoing education in individual, group, and clinic settings. �Provides Diabetes Self-Management Education and Support (DSMES) within the scope of practice of the RN/RD as outlined by the American Diabetes Association Recognized Education Program Standards and according to department curriculum. �Creates and maintains diabetes education materials used for individual and group education. �Acts as a liaison between the patient/family and MD/APRN by providing ongoing patient support via telephone, e-mail, and EMR-based messaging as needed for comprehensive glucose management, insulin adjustments per department-approved protocol, and for general concerns from patients/families/school nurses that occur in-between office visits. �Works closely with school clinic staff of surrounding school systems to provide diabetes education focused on keeping children safe at school and assists with completion of Diabetes Medical Management Plans for all patients as required by schools. �Provides training and ongoing education for diabetes technology devices (insulin pumps, continuous glucose monitors, smart insulin pens) and becomes certified by each individual diabetes technology company to train and educate patients/families on use of these devices. �Participates in ongoing quality improvement initiatives within the department as well as within programs through the State of Florida Department of Health. �May choose to participate in diabetes camps throughout the year as part of the camp�s medical staff. �Stays knowledgeable about the on-going research studies at the University of Florida and helps identify and recruit appropriate subjects for these studies. �Must have excellent communication skills and be able to work well in a multidisciplinary environment with proven ability to problem-solve.The Diabetes RD additionally provides Medical Nutrition Therapy (MNT) tailored to diabetes, metabolic, and weight management following the Nutrition Care Plan process to provide high quality nutritional care demonstrating knowledge of the principles of growth and development of the pediatric patient. The RD also assists with precepting student interns from the University of Florida.
Qualifications

Minimum Education and Experience Requirements:

Diabetes Nurse Minimum Education and Experience Requirements:

  • Graduate of an accredited School of Nursing with BSN.
  • Current licensure by the State of Florida as a Registered Nurse or eligible for same.
  • Current certification as a Certified Diabetes Care and Education Specialist (CDCES) through the Certification Board for Diabetes Care and Education required within 1 year of hire.
  • Basic Life Support certification required or obtained at first opportunity with employment.
  • Prefer 3-5 years clinical experience including familiarity with diabetes education. 

 Motor Vehicle Operator Designation: Employees in this position: will not operate vehicles for an assigned business purpose.

Diabetes Dietitian

Minimum Education and Experience Requirements:

  • Graduate of an accredited four-year college or university with major course work in clinical dietetics. Masters Degree preferred.
  • Current and valid registration as issued by the Commission on Dietetic Registration, the credentialing board of the Academy of Nutrition and Dietetics.
  • Current licensure by the State of Florida as a dietitian/nutritionist or be eligible for same.
  • Current certification as a Certified Diabetes Care and Education Specialist (CDCES) through the Certification Board for Diabetes Care and Education required within 1 year of hire.
  • Prefer 3 � 5 years clinical experience including familiarity with diabetes education.
