Key Responsibilities
  • Conduct comprehensive psychosocial assessments of patients and their families upon their admission and periodically as clinically warranted. Identify barriers to care, including environmental and psychological factors that impact treatment.
  • Collaborate with interdisciplinary healthcare teams to develop and implement comprehensive treatment plans that address both psychosocial and medical needs, while coordinating referrals to additional services.
  • Provide empathetic support to patients and families facing challenges due to illness, including crisis intervention and grief counseling, empowering them to adapt to changes in their health status.
  • Manage a diverse caseload, independently planning and executing treatment strategies that are responsive to the varying levels of clinical acuity.
  • Engage in proactive case finding for patients who may need psychosocial support, actively contributing to team discussions and interdisciplinary rounds.
  • Deliver psychoeducation to patients and families, focusing on how to navigate the psychosocial aspects of diagnoses and treatment, especially during transitions from youth to adult services.
  • Participate in quality assurance initiatives aimed at improving social work services, contributing to policy reviews, and establishing best practices within the organization.
  • Oversee and provide clinical supervision to Master of Social Work (MSW) students and interns, fostering their professional growth and development.
  • Document all interactions and progress through electronic medical records while adhering to confidentiality and regulatory standards.
Patient Population / Client Focus

Qualifications
  • A valid professional license in California as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC).
