Position Overview

Evergreen Physical Therapy Services is seeking a dedicated Physical Therapist (PT) to join our dynamic rehabilitation facility in Hartford, Connecticut. This full-time position offers a competitive salary ranging from USD 100,000 to 120,000 annually, along with a comprehensive benefits package. Key Responsibilities
  • Conduct thorough patient evaluations, which include reviewing medical histories, performing specific assessments, and developing appropriate treatment plans tailored to individual needs.
  • Effectively communicate evaluation results and treatment strategies to patients and their families, involving them in goal setting throughout the rehabilitation process.
  • Implement and adjust treatment protocols based on patient progress and evolving healthcare standards, ensuring the highest level of care.
  • Consult with multidisciplinary healthcare teams as necessary, and document any changes in a patient’s condition requiring updates to the care plan.
  • Manage discharge planning and follow-up procedures, ensuring clear and accurate documentation of patient interactions, progress notes, and treatment goals.
  • Maintain an effective schedule, stay punctual for appointments, participate in departmental meetings, and contribute to community outreach initiatives.
  • Support departmental goals and compliance with healthcare regulations, engaging in initiatives to optimize patient access and care quality.
  • Actively pursue professional development through continuing education, while serving as a mentor for student interns and new employees.
  • Promote a positive patient experience by ensuring flexible patient access and adapting to diverse healthcare needs.
Patient Population / Client Focus

You will have the opportunity to work with a diverse array of patients ranging from young athletes to seniors, each requiring tailored physical therapy interventions. Our services span acute rehabilitation post-surgery, recovery following injuries, and chronic pain management, allowing you to engage with a broad spectrum of cases. Your role will directly influence the rehabilitation journey of these individuals, focusing on their unique needs and goals.

Qualifications
  • A Bachelor's Degree in Physical Therapy or a related field is required, with a Doctorate in Physical Therapy (DPT) preferred.
  • A valid state license to practice physical therapy in Connecticut is mandatory.
  • Current certification in Basic Life Support (BLS) from the American Heart Association is required.
  • A strong commitment to providing compassionate care and improving patient outcomes through effective therapeutic interventions.
Preferred Experience
  • A minimum of two years’ experience as a Physical Therapist, preferably in a rehabilitative or outpatient setting.
  • Experience with electronic medical records (EMR) systems and utilization of physical therapy documentation software is highly desirable.
